SPECIALIZED CARE PROGRAMS

To ensure our residents maintain the highest level and standard of care, Vermont Square offers many specialized programs to benefit our residents such as:

Specialized Nutritional Therapy:
Our Registered Nursing staff are trained to deliver G-tube feeding for residents when oral feeding can no longer be done. We also provide alternative therapies for hydration when needed, such as Hypodermoclysid therapy, which helps to hydrate those residents who require increased fluid intake and do not want to be transferred to hospital. Our dietary staff and dietitian are also available to ensure that the nutritional needs of each resident are met.

Restorative Care Program:
The program offers a variety of services to improve or maintain the independence of our residents including restorative dining and adaptive equipment to assist with activities of daily living. We also have access to an Occupational Therapist and Speech Language Pathologist.

Foot Care:
We provide on-site foot care services by a qualified Registered Practical Nurse who visits the home weekly. All residents are assessed upon admission and then quarterly and routinely as needed for foot conditions and for appropriate footwear. There is no extra cost to the resident. Referrals for Podiatry can be arranged as required.

Wound Care Management Program:
An on-site Wound and Skin Care Coordinator helps in the prevention and management of wounds. The Skin Care Coordinator assesses each resident that has skin breakdown on admission and weekly thereafter. In addition,the coordinator monitors the effectiveness of prescribed treatment. We also access the consultant services of an ET Nurse who visits monthly to monitor the healing of the wounds and request specialized services through the Ministry of Health Intensity Needs Program as required. The Enterostomal Therapy Nurse mentors the Wound and Skin Care Coordinator on innovative treatments in wound healing that will benefit the needs of our residents.

End of Life Care Program:
Our End of Life Care Program is designed to provide an interdisciplinary approach to palliative care services in long term care homes. Our staff are equipped with the knowledge and skills to effectively provide care to residents who are living with, or dying from progressive or chronic life threatening conditions. The end of life philosophy focuses on providing comfort, managing symptoms and maitaining quality of life.
